This service, subsidised by the Government Social Welfare Department, was established in 1999 and expanded into Integrated Home Care Services in 2003. Situated in Wan Chai, it offers home-based caring and professional community care services. Through a range of home-based support services, the service assists frail elderly persons in aging in place, while also providing various forms of support and training for caregivers to relieve caregiving stress.
Service for ordinary and frail cases:
Ordinary Cases: elderly persons aged 60 or above, persons with disabilities, and individuals and families with social needs living in the community
Frail Cases: frail elderly persons aged 60 or above living in the community and who are assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services

The service team will tailor the service content and frequency according to the individual needs of service users.
